Covered Conditions
- Medicare Advantage Plans: coverage is limited to chronic low back pain lasting 3 months or longer.
- Commercial Plans: coverage is limited to musculoskeletal pain conditions, including pain or dysfunction involving joints, muscles, nerves, tendons, ligaments, discs, and related neuromusculoskeletal conditions. Some insurance policies may also cover nausea due to chemotherapy or pregnancy. Some insurance policies may cover headaches.
All other diagnoses and wellness visits are considered self-pay.
